Student Medical Information

If your child has a serious, life threatening medical condition please fill out a Medical Alert Form and bring it to the school office. These need to be updated yearly so our information is current. Please include a recent photo of your child as well. If your child requires medication while at school, please complete a Medication Administration form and provide offices staff with the medication and instructions.
